| 1. Narconon is a residential drug and alcohol rehabilitation facility. We provide a home like atmosphere for the treatment and rehabilitation of alcohol and chemical dependencies. 2. Narconon® does not discriminate on the basis of race, colour, natural origin, religion, sex or background. 3. General services that will be provided by the facility to all residents are: a. Lodging. |

| 4. Health services that will be provided by the facility to all residents are: a. Assistance with obtaining doctor / dentist as needed. b. Arrangements for keeping doctor / dentist appointments. c. First Aid as needed. 5. Centrelink requirements: a. Residents are to be on fortnightly Centrelink benefits whilst on site. b. Narconon Sydney will assist the residents with all data required for them to obtain benefits prior to starting the program. c. Narconon® Sydney will be the nominee for all Centrelink payments. d. If Narconon Sydney is not made the nominee for Centrelink payments, then the resident is required to pay the fee for food and lodging, this amount being $180.00 per week. e. In liaison with the student, Narconon® will process and make necessary adjustments as per Centrelink requirements to commence payments. Documentation will be required ie certified copy or original birth-certificate, passport, driver’s licence, medicare card, bank debit card, credit card. f. Narconon Sydney will issue the balance of the fortnightly Centrelink payments to the resident in weekly payments, for spending money (which is any monies over the $180 per week food and lodging fee). g. If for any reason, the resident is not eligible for Centrelink benefits, then the resident is responsible for payment of food and lodging. This amount is $180.00 per week. Any additional living expenses are to be funded by the resident. 6 Narconon Sydney Sydney policies are as follows: a. We are a non-profit organisation. b. Services must be paid for in advance. c. If a cheque is returned to us dishonoured, the resident may be removed immediately and the resident or the resident’s representative will be liable for the amount of the cheque plus a $15.00 handling charge. d. A thirty (30) day written notice will be given in advance on any change of cost of service. 7. Narconon Sydney repayment policies are as follows: a. If the resident leaves the facility of their own will and without any notice, their bed will be held at the usual price for a period of one week or until they (or their representative) give notice that they do not want the bed held. b. All goods, that belong to students who are no longer at Narconon Sydney, will be placed in storage for 2 weeks only. After 2 weeks the goods will be disposed of. Payment for return of the goods such as postage etc must be paid in advance. c. All repayments of unused fees will be issued within 6 months to the person who originally paid only. d. All repayment requests must be made in writing no later than 30 days after the resident leaves the program. e. The resident’s representative must sign all repayment requests. The only exception is if the resident has paid all the fees for the program himself. f. Before a repayment will be given, the resident must participate in an interview with an administrator to go over the reasons why he/she wants to leave the program. If after the interview the resident still wants to leave the program and receive a payment of unused money, one will be given as long as the requirements of this repayment policy are met. g. Narconon Sydney will not start to process a repayment until all of the conditions of this repayment policy have been completed. h. There will be no refund if a person is removed from or leaves the program having violated the terms agreed to in number ten (#10) of this agreement. i. If any student uses drugs or alcohol while enrolled on the Narconon program, right to any repayment is considered null and void. j. Any student who offers drugs or alcohol to any other enrolled student can be terminated from the Narconon program, and any right to refund will be null and void. k. The initial payment of Phase 1 is non-refundable. 8. The facility will not be responsible for any money, valuables, or personal items etc, brought into the facility unless those items are given to an administrator for safekeeping and the administrator accepts them as such. 9. Readmission policy for residents who leave the program of their own volition: Each case will be evaluated on it’s own basis. The resident by wanting to re-enter the program will be evaluated on his previous performance in the program and his willingness to do the program. However, no residents will be allowed back inside the facility while they are under the influence of drugs or alcohol, for twenty-four hours. © 2000 Narconon Sydney.
